Not a WVUChart user? When registering online, just click on the appropriate consent agreement, fill it out, and fax it to the number on the form. After receiving secure access, you will get an email with an initial username and password.

Physicians may also refer patients over the phone by calling 855-WVU-CARE, or by faxing a referral form.

We recognize that you are the point of contact for your patient’s ongoing care after being seen at the WVU Cancer Institute. Should you need us again, we’ll remain a resource for you and your patient to assist in coordinating any long-term care management.
